CONSTRUÇÃO E VALIDAÇÃO DE UM FLUXOGRAMA PARA ARTICULAÇÃO DA SAÚDE PÚBLICA E SUPLEMENTAR SOB A ÓTICA DA INTEGRALIDADE DO CUIDADO

Resumo:
O estudo teve como objetivo construir e validar um fluxograma para articulação da saúde pública e suplementar sob a ótica da integralidade do cuidado, considerando que atualmente a integração dos setores acontece na perspectiva de ressarcimento ao SUS. Trata-se de um estudo de natureza metodológica, realizado de Setembro a Novembro de 2018, composto por três fases: FASE I – etapa qualitativa com a realização de entrevistas semiestruturada com 05 participantes que representaram os setores público e suplementar da saúde; FASE II - etapa de elaboração do fluxograma a partir das propostas recebidas; e FASE III – etapa relacionada à Validação de Conteúdo e Aparência. Após a leitura e codificação do material obtido a partir das entrevistas, foram realizadas análises de conteúdo categorial clássica e com suporte do NVivo, sendo possível extrair dados importantes para compor a primeira versão do fluxograma, que reuniu, em uma representação gráfica, todas as cadeias dos processos de trabalho propostos pelos participantes para atender o objetivo deste estudo. A primeira versão da ferramenta passou pela validação de conteúdo por meio da avaliação de 03 juízes de conteúdo (pesquisadores/docentes na área de saúde Coletiva, Gestão em saúde e/ou validação de instrumentos) e 04 juízes técnicos (profissionais com experiência na área objeto do estudo) e pela validação de aparência que foi realizada pelos 03 juízes de conteúdo (pesquisadores/ docentes) supracitados. O resultado das validações trouxe escores de 100% de concordância dos Juízes de Conteúdo (IVC = 1,00) e classificou o fluxograma como “superior” conforme medida do instrumento SAM. A partir do retorno das avaliações dos especialistas, os itens aceitos no fluxograma, conforme os consensos das respostas ao nível de concordância, foram mantidos. Os itens que tiveram apontamentos, foram ajustados, gerando a versão final do fluxograma. A ferramenta elaborada se constitui em proposta inovadora para o Sistema de Saúde e pretende contribuir para a sistematização de rotina para articulação da saúde pública e saúde suplementar sob a ótica da integralidade do cuidado, podendo também trazer benefícios que vão desde a ampliação na oferta de leitos no SUS, até o fomento para as iniciativas e estratégias que melhorem gradativamente os desempenhos assistenciais das Operadoras de Planos de Saúde.

Abstract:
The goal of this work was to construct and validate a flowchart for the articulation of public and supplementary health care from the point of view of integral care, considering that currently the integration of these sectors happens only in the perspective of the refunding program for SUS. It is a methodological study, carried out from September to November 2018, composed of three phases: PHASE I - qualitative stage with semi-structured interviews with 05 participants who represented the public and supplementary sectors of health care; PHASE II - stage of producing the flowchart based on the proposals received; PHASE III - stage related to content and appearance validation. After reading and coding the material obtained from the interviews, classic categorical analysis and analysis with NVivo support were performed, thus making possible to extract important data to compose the first version of the flowchart, which gathered, in a graphic representation, all the chains of the processes proposed by the participants to fulfill the goal of this study. The first version of the tool went through content validation through the evaluation of 3 content judges (researchers/teachers in of Collective Health care, Health care Management and/or validation of instruments) and 04 technical judges (professionals with experience in the subject of the study) and by the appearance validation that was performed by the three content judges (researchers/teachers) mentioned above. The results of the validations brought scores of 100% concordance from the Content Judges (IVC = 1.00) and ranked the flowchart as "superior", as measured by the SAM instrument. From the feedback of the experts’ evaluations, the items accepted in the flowchart, according to the consensus of the answers consent, were kept. The items that had notes were adjusted, creating the final version of the flowchart. The tool that was prepared composes an innovative proposal for the Health care System, with a view to contribute to the routine systematization for articulation of public health care and supplementary health care in the perspective of the integrality of care, also being able to bring benefits that range from the expansion of provision of hospital units in SUS to the promotion of initiatives and strategies that gradually improve the performance of the Health care Insurance Operators.
