SABERES E PRÁTICAS DOS TRABALHADORES DA SAÚDE PARA HUMANIZAÇÃO DO CUIDADO EM MATERNIDADE PÚBLICA

Resumo:
O serviço de maternidade situada em hospitais públicos, com destaque para os indicadores de mortes maternas e neonatais elevados, e insatisfação de usuárias quanto à humanização do cuidado se configuram como fatores desgastantes, para o cuidado e suas relações, podendo resultar em violência institucional. Nessa casuística, a Política Nacional de Humanização (PNH) surgiu em 2003 para ser implantada em todos os âmbitos da saúde, com o objetivo de provocar inovações nas práticas assistenciais, bem como nas formas de gerir e produzir saúde. Objetivou-se identificar os saberes e práticas dos trabalhadores de saúde que se relacionam à humanização do cuidado, no que concerne à política nacional de humanização, no contexto materno hospitalar; realizar ações de ensino-aprendizagem a partir das lacunas de conhecimento demonstradas pelos participantes sobre humanização e a referida política. Método: Utilizou-se a pesquisa-ação, através da formação Comunidade Ampliada de Pesquisa (CAP). As etapas da pesquisa-ação contemplaram a caracterização do perfil dos sujeitos através de questionário semiestruturado; diagnóstico situacional com a aplicação do Pré-Teste; etapa da ação com aplicação da intervenção educativa utilizando-se a ferramenta metodológica circuito; e a etapa de avaliação explicitando os resultados obtidos com o Pós-Teste. Teve como cenário o Hospital e Maternidade Zulmira Sedrin de Aguiar, município de Cedro-CE, referência regional em maternidade pública. Os sujeitos foram 18 trabalhadores implicados diretamente na assistência à maternidade. Para detalhamento e maior fidedignidade do conteúdo obtido foi utilizado diário de campo. A pesquisa obedeceu à Resolução 466/12 e foi aprovada pelo Comitê de Ética, conforme parecer 52709316.0.0000.5055. Resultados e Discussões: A caracterização dos sujeitos mostrou a predominância do gênero feminino com 77,77%; 66,66% são técnicos de enfermagem e 22,22% enfermeiros; a maioria se apresenta com idade entre 32 a 45 anos; e tempo de atuação no cenário de estudo de 1 a 5 anos. Os resultados evidenciaram conhecimentos subjetivos dos profissionais sobre humanização através de condutas que os mesmos caracterizavam como ações humanizadoras. Com vistas à PNH os sujeitos da pesquisa relacionaram à valorização dos direitos das usuárias. A CAP permitiu o compartilhamento de ideias e propostas positivas de mudança, contemplando a
necessidade da efetivação da educação permanente e adesão dos trabalhadores a essa perspectiva, tornando-se assim um caminho metodológico possível, exequível para todos os envolvidos. Considerações finais: A pesquisa permitiu transformações reflexivas acerca das práticas de humanização e dos princípios para operacionalização da PNH, afirmando exitosamente a proposta da CAP, para fomentar novas práticas e saberes coletivos, promovendo a interação nas relações de trabalho, e melhoria na maternidade pública e suas usuárias.

Abstract:
Maternity service in public hospitals, with emphasis on indicators of high maternal and neonatal deaths, and users' dissatisfaction with the humanization of care are seen as exhausting factors for care and their relationships, which may result in institutional violence. In this series, the National Humanization Policy (HNP) was created in 2003 to be implemented in all areas of health, with the aim of provoking innovations in healthcare practices, as well as in the ways of managing and producing health. The aim of this study was to identify the knowledge and practices of health workers related to the humanization of care, as regards the national policy of humanization, in the maternal hospital context; to carry out teaching-learning actions based on the knowledge gaps demonstrated by the participants about humanization and the said policy. Method: Research-action was used through the Expanded Community Research (CAP) training. The steps of the action research contemplated the characterization of the subjects' profile through a semi-structured questionnaire; situational diagnosis with the application of the Pretest; stage of action with application of educational intervention using the methodological tool circuit; and the evaluation stage explaining the results obtained with the Post-Test. The scenario was the Hospital and Maternity Zulmira Sedrin de Aguiar, municipality of Cedro-CE, regional reference in public maternity. The subjects were 18 workers directly involved in maternity care. For the detailing and greater trustworthiness of the obtained content, field diary was used. The research complied with Resolution 466/12 and was approved by the Ethics Committee, as it appears 52709316.0.0000.5055. Results and Discussion: Characterization of the subjects showed the predominance of the female gender with 77.77%; 66.66% are nursing technicians and 22.22% are nurses; the majority are aged between 32 and 45 years; and time of performance in the study scenario of 1 to 5 years. The results evidenced the subjective knowledge of the professionals about humanization through behaviors that they characterized as humanizing actions. With regard to PNH, the subjects of the research related to the valorization of the users' rights The CAP allowed the sharing of ideas and positive proposals for change, contemplating the need for permanent education to be put in place and workers' adherence to this perspective, thus becoming a possible methodological path, feasible for all those involved. Final considerations: The research allowed reflective transformations on the humanization practices and
principles for the operationalization of the HNP, successfully affirming the proposal of the CAP, to foster new practices and collective knowledge, promoting interaction in labor relations, and improvement in public maternity and their users.
