CUSTO-EFETIVIDADE DO SERVIÇO DIAGNÓSTICO POR IMAGEM EM POLÍCLÍNICAS, COMPARANDO A EMISSÃO DE LAUDOS VIA MEDICINA CONVENCIONAL X TELEMEDICINA: SUBSÍDIO À TOMADA DE DECISÃO GERENCIAL

A organização do Sistema Único de Saúde (SUS) pressupõe a constituição de uma rede de serviços pautada nos princípios da hierarquização e regionalização, possibilitando um conhecimento maior dos problemas de saúde da população de cada área, permitindo a realização de ações de vigilância epidemiológica, sanitária, ambiental e educação em saúde, além do acesso ao conjunto das ações de atenção especializada ambulatorial e hospitalar em todos os níveis de complexidade. O Programa de Expansão e Melhoria da Assistência Especializada em Saúde no Estado do Ceará (Proexmaes), do Governo do Estado do Ceará, surgiu com o objetivo de otimizar a gestão da rede de atenção secundária, colocando em prática o custeio compartilhado entre os municípios e o estado para funcionamento de unidades de saúde, dentre elas as Policlínicas, por meio da criação dos Consórcios Públicos. Contudo, a área da atenção especializada ainda tem sido um dos grandes problemas enfrentados nas regiões de saúde, devido à escassez de exames e consultas especializadas, seja pela ausência do profissional especialista, seja pela insuficiência de recursos financeiros. Destaca-se, nesse ínterim, a prestação de serviços de saúde à distância com o apoio na Saúde Digital (e-Saúde) para fins de minimização das barreiras geográficas existentes. Em se tratando de exames de diagnóstico por imagem, há a necessidade de laudos médicos com qualidade e em tempo hábil a menores custos. Assim, o objetivo deste estudo foi realizar uma análise de custo-efetividade do diagnóstico por imagem em Policlínicas Tipo II regidas por Consórcio Público no Ceará, tendo como alternativas a emissão de laudos fornecidos pela medicina convencional e Telemedicina. A metodologia foi exploratória com abordagem quantitativa, utilizando como método a avaliação econômica em saúde. O período estudado foi o ano de 2018. Os resultados mostraram a alternativa mais vantajosa a Telemedicina para os exames de Raio X (R$326,56) e Eletrocardiograma (R$4,91) e a alternativa de Medicina Convencional para os exames de Tomografia (R$377,97/ ICER R$2.150,66) e Eletroencefalograma (R$379,30/ ICER R$12.212,83). Verificou-se que apenas duas unidades de saúde alimentaram o sistema de custos da SESA, inviabilizando um gerenciamento de custos efetivo e, por consequência, uma análise econômica limitada. Por fim, foi elaborado um Termo Normativo para fins de padronização e utilização do sistema de custos da SESA pelos gestores das Policlínicas regidas por Consórcios Públicos de Saúde do Estado do Ceará. Abstract:
The organization of the Unified Health System (SUS) presupposes the
constitution of a service network based on the principles of
hierarchization and regionalization, allowing a greater knowledge of the
health problems of the population of each area, allowing the
implementation of epidemiological and sanitary surveillance actions,
environmental and health education, as well as access to the set of
outpatient and hospital specialized care actions at all levels of
complexity. The Program for the Expansion and Improvement of Specialized
Health Care in the State of Ceará (Proexmaes), of the Ceará State
Government, came up with the objective of optimizing the management of
the secondary care network, putting into practice the shared costing
between the municipalities and the state for the operation of health
units, including the Polyclinics, through the creation of Public
Consortia. However, the area of specialized care has still been one of
the major problems faced in health regions due to the scarcity of
specialized examinations and consultations, either due to the absence of
the specialist professional or the lack of financial resources. In the
meantime, the provision of distance health services with support in
Digital Health (e-Health) to minimize existing geographical barriers
stands out. In the case of diagnostic imaging exams, there is a need for
quality and timely medical reports at lower costs. Thus, the objective
of this study was to perform a cost-effectiveness analysis of imaging
diagnosis in Type II Polyclinics governed by the Public Consortium in
Ceará, having as alternatives the emission of reports provided by
conventional medicine and Telemedicine. The methodology was exploratory
with quantitative approach, using as method the economic evaluation in
health. The study period was 2018. The results showed the most
advantageous alternative to Telemedicine for X-ray (R$ 326.56) and
Electrocardiogram (R$ 4.91) and the Conventional Medicine alternative
for Tomography (R$377,97/ ICER R$2.150,66) and Electroencephalogram
(R$379,30/ ICER R$12.212,83). It was found that only 02 health
facilities fed SESA's cost system, making effective cost management
impossible and, as a result, limited economic analysis. Finally, a
Normative Term was prepared for standardization and use of the SESA cost
system by the managers of the Polyclinics governed by Public Health
Consortia of the State of Ceará. Keywords: Telemedicine.
