Resumo:
As diretrizes político-assistenciais defendidas pelo Movimento Brasileiro de Reforma Psiquiátrica-MBRP vislumbram uma real mudança nos serviço de atenção à saúde mental, que se reflete em significativas transformações nas práticas e nos cenários de atuação. O modelo hospitalocêntrico e o hospital psiquiátrico asilar poderão ser efetivamente superados se a rede substitutiva for instalada, garantindo segurança continuada à população. O MBRP vem vivenciando, não apenas falhas e lacunas, mas também redução da velocidade de crescimento e regressão geral no processo de avanços e conquistas. Com base nisso a presente pesquisa caracterizada como um estudo de caso, analítico-reflexivo e de natureza qualitativa, teve como objetivo acompanhar ao modo etnográfico a transformação do modelo de atenção psiquiátrica em município de referência regional em saúde mental, como decorrência da decisão de fechamento do hospital psiquiátrico, a fim de questionar a realidade que vem sendo desenvolvida no campo da saúde mental no Brasil e no Ceará. De acordo com os resultados a longa existência HCSSTJ e a implantação superficial e truncada das mudanças preconizadas pelo MBRP explicam a permanência dos preconceitos em relação à doença mental e as resistências em aceitar as mudanças. A ineficiente atuação da ESF enquanto dimensão de práticas que incorpore a saúde mental à saúde da família, no nível da atenção primária foi outra vulnerabilidade encontrada. Falta suporte da gestão para as equipes se consolidarem como parceiras na efetivação do cuidado em saúde mental para os territórios sociais. Portanto, a permanência institucionalizado do paciente de maneira desumana, bem como a implantação da rede assistencial com financiamento inadequado e sem consonância com as principais normas e diretrizes da politica de reforma. É preciso requalificar a assistência, qualificar os profissionais atuantes no campo e desmistificar a cultura do isolamento da pessoa com transtorno mental. O que se deve ser estimulado não é a reprodução da instituição como asilar, manicomial, e sim que ela consiga promover assistência, garantindo respeito, estimulando a autonomia, enquanto as forças políticas da sociedade sejam capazes de instalar a rede de atenção psicossocial territorial integrada à saúde coletiva, às políticas de direitos humanos e de inclusão social.

Abstract:
The political care guidelines advocated by MBRP spot a real shift in attention to mental health service , which is reflected in significant changes in the practices and performance scenarios . The hospital-centered model and asylum psychiatric hospital can be effectively overcome if the replacement network is installed, ensuring continuous security to the population. The MBRP has experienced not only faults and shortcomings, but also reducing the rate of growth and overall regression in the advances and achievements process. On this basis the present research characterized as a case study, analytical and reflective and qualitative, aimed to monitor the ethnographic mode transformation of psychiatric care in the municipality of regional reference model in mental health as a result of the closure decision psychiatric hospital to question the reality that is being developed in the field of mental health in Brazil and Ceará. According to the results of the long existence HCSSTJ and superficial and truncated the changes advocated by MBRP deployment explain the persistence of prejudices regarding mental illness and the resistance to accept the changes. Inefficient performance of ESF as a dimension of practice that incorporates mental health to family health at the level of primary care was another vulnerability found. SUS lack of support for teams to consolidate as partners in effecting mental health care to social territories. Therefore, the institutionalized patient stays inhumanely, as well as the implementation of the assistance network with inadequate funding and without compliance with key standards and guidelines of the policy reform. You need to upgrade the service, qualify the professionals working in the field and demystify the culture of isolation of the person with mental disorder. What should be encouraged is not the reproduction of the institution as asylum, mental hospital, but she can promote assistance , ensuring compliance , encouraging autonomy , while the political forces of society are able to install the network of territorial integrated psychosocial care to public health , the politics of human rights and social inclusion .
