ANÁLISE DA PRODUÇÃO DO CUIDADO ÀS CRIANÇAS
SUBMETIDAS A IMPLANTE COCLEAR EM UM SERVIÇO TERCIÁRIO DE SAÚDE AUDITIVA DE FORTALEZA

O Programa de Implante Coclear, no Brasil, cresce a cada dia, com a ampliação pelo Ministério da Saúde de novos serviços habilitados. É um tratamento de alto custo, ainda não universalmente constituído, afunilando a grande demanda de usuários que necessitam dessa tecnologia avançada. Surgiu então a necessidade de buscar um conhecimento maior sobre esse tratamento que, segundo seus critérios de inclusão, traria avanços significativos para o desenvolvimento mais adequado de crianças surdas. O objetivo geral foi analisar a produção do cuidado às crianças submetidas a implante coclear em um serviço terciário de saúde auditiva no município de Fortaleza. Como objetivos específicos, identificar as formas de acesso dos usuários e o cuidado às crianças submetidas à implante coclear, na visão de suas famílias e dos profissionais de saúde. Trata-se de um estudo de natureza qualitativa, realizado no ambulatório de implante coclear do Hospital Geral de Fortaleza, no período de agosto a outubro de 2013. Os participantes foram quinze responsáveis pelas crianças que realizaram implante coclear e cinco profissionais do serviço de implante coclear. Utilizamos a entrevista semiestruturada com os familiares e uma oficina com os profissionais de saúde para construção de um Fluxograma Analisador, proposto por Mehry, para descrição do fluxo de atendimento do usuário no serviço. A análise de conteúdo proposta por Flick (2009) foi utilizada para as entrevistas e resultou em duas temáticas centrais e suas respectivas categorias. A primeira temática encontrada foi: o caminho desde a descoberta da surdez até o Serviço de Implante Coclear, com as categorias, descoberta da surdez, busca pelos serviços de saúde e o acesso ao Serviço de Implante Coclear do HGF. A segunda temática: a produção do cuidado à criança submetida à cirurgia de implante coclear, que dividiu-se em três categorias: O cuidado pela equipe de saúde: fluxos de atendimento no serviço de implante coclear; O cuidado pela família: participação, envolvimento e corresponsabilização; e, percepções diante dos resultados do tratamento: expectativas e a realidade. O Fluxograma Descritor revelou o conhecimento dos profissionais sobre a rotina para entrada e acolhimento dos usuários no Serviço de Implante Coclear do Hospital e identificou a demanda espontânea como a principal forma de acesso desses usuários, não sendo referenciados por um sistema de regulação. Os profissionais do estudo demonstram uma profunda vinculação com as famílias e os usuários, através de práticas acolhedoras, responsabilização e comprometimento. Observamos que os familiares reconhecem a importância do implante coclear e do tratamento fonoaudiológico como primordial para o desenvolvimento linguístico dessa criança e que sem a continuidade desse cuidado, todo o investimento terá sido em vão. Os familiares são participativos e envolvidos com o tratamento de seus filhos, são cientes que fazem parte integrante do processo e acreditam que fizeram a melhor escolha pelos seus filhos. Abstract:
The cochlear implant program, in Brazil, grows every day, with the extension by the Ministry of Health of new enabled services. It’s a high-cost treatment, still not universally constituted, tapering the great demand of users who require this advanced technology. Then came the necessity to seek a greater knowledge about this treatment that, according to its criteria for inclusion, would bring significant improvements to the most appropriate development of deaf children. The overall objective was to analyze the production of care for children subjected to cochlear implant in a tertiary auditory health service in the city of Fortaleza. As specific objectives, identify the forms of access for users and care to children undergoing cochlear implant, in view of their families and health professionals. This is a qualitative study, carried out in the cochlear implant clinic of General Hospital of Fortaleza, in the period of August to October 2013. The participants were 15 children’s responsible who were undergoing for the cochlear implant and five professionals in the service of cochlear implant. We used the semi-structured interview with family members and a workshop with health professionals to build a Flowchart Analyzer, proposed by Mehry for description of the flow of the user in the service. Content analysis proposed by Flick (2009) was used for the interviews and resulted in two central themes and their respective categories. The first theme that was found: the path since the discovery of deafness to the cochlear implant service, with categories, discovery of deafness, search for health services and access to the service of cochlear implant of HGF. The second theme: the child care production subjected to the cochlear implant surgery, which was divided into three categories: care by health team: customer service streams of cochlear implant; care for the family: participation, involvement and co-responsibility; and, perceptions on the results of the treatment: expectations and reality. The Descriptor Flowchart revealed the knowledge of professionals about the routine for entry and reception of cochlear implant service users of the Hospital and identified the spontaneous demand as the primary way to access these users, it’s not being referenced by a regulation system. The professionals of the study demonstrate a deep linking with the families and the users, through cozy practices, accountability and commitment.We observed that the relatives recognize the importance of the cochlear implant and speech therapies as primordial treatment for the linguistic development of this child and that without the continuity of this careful, all the investment will have been in vain. The relatives are participatory and involved with the treatment of their children, are aware that they are an integral part of the process and believe that they made the best choice for their children.
