PROPRIEDADES PSICOMÉTRICAS DA ESCALA DE SUPORTE SOCIAL DO MEDICAL OUTCOMES STUDY (MOS) EM IDOSOS DO NORDESTE DO BRASIL

O Suporte social é um importante determinante de boas condições de saúde. A versão em português de uma das escalas de suporte social, a do Medical Outcomes Study (MOS) foi validada numa população de funcionários de uma universidade publica do Rio de Janeiro. O objetivo deste trabalho foi avaliar algumas propriedades psicométricas da escala de suporte social do MOS, numa população de idosos da cidade de Juazeiro do Norte, Ceará, com características socioculturais diferentes daquela na qual a escala foi validada no Brasil. Foram realizados dois inquéritos transversais da uma amostra aleatória de base populacional, de 208 idosos, através de aplicação da escala por entrevistador, com intervalo de 15 dias entre as duas aplicações. Foi avaliada a consistência interna através do Coeficiente Alfa de Cronbach, e a confiabilidade teste-reteste através do Kappa Ponderado, do Coeficiente de Correlação (CCI) Intraclasse e de modelos log-lineares. Na amostra, 75% eram mulheres, a idade variou entre 60 e 101 anos, 41% tinha idade inferior a 70 anos, e 55% não sabiam ler e escrever. A consistência interna da escala foi elevada (Alfa de Cronbach maior que 90%), a confiabilidade teste-reteste foi baixa, quando avaliada pelo Kappa Ponderado (0,23 a 0,50) e pelo CCI (entre 0,23 e 0,53). Mas, a confiabilidade teste-reteste estimada pelos modelos log-lineares foi satisfatória para 17 dos 19 itens. Foi observado um alto nível de percepção de suporte social na população estudada, com uma média do escore total de 84,7(IC 95%: 82,5-86,8). Também, observou-se uma correlação direta e significativa entre a soma do número de amigos e familiares e o nível de suporte social. A escala MOS aplicada por entrevistador é um instrumento capaz de medir de modo reproduzível o suporte social de populações de idosos com baixa escolaridade.
Social support is an important factor of good health condition. The version in Portuguese of one of the social support scales, from the Medical Outcomes Study (MOS), was validated in a population of employees of a public university in Rio de Janeiro. The goal of this study was to evaluate some psychometric properties of the MOS social support scale in a population of elderly people in the city of Juazeiro do Norte, Ceará, with sociocultural characteristics that differ from those in which the scale was validated in Brazil. Two transversal inquiries were performed in a random sample of 208 elderly people, through the application of the scale by the interviewer, with a gap of 15 days between the two applications. Internal consistency was evaluated through the Alpha Cronbach's Coefficient, and the test-retest reliability, through the weighted Kappa, the Intraclass Correlation Coefficient (ICC), and log- linear models. In the sample, 75% were women, the age varied from 60 to 101 years, 41% were less than 70 years old, and 55% were illiterate. The internal consistency of the scale was high (Alfa de Cronbach's coefficient greater than 90%), the test- retest reliability was low when evaluated by the Weighed Kappa (which varied from 0.23 and 0.50) and by the ICC, which varied from 0.23 and 0.53. However, the test- retest reliability estimated by the log-linear models was satisfactory for 17 out of the 19 items. It was possible to observe a high level of social support perception within the studied population, with a total average score of 84.7(IC 95%: 82.5-86.8). It was also noted that there was a direct and significant correlation between the amount of friends and relatives and the level of social support. The MOS scale applied by an interviewer is a tool capable of measuring - in reproducible mode - the social support of populations of elderly people with less schooling.
