A INTEGRAÇÃO ENSINO-SERVIÇO E COMUNIDADE NA GRADUAÇÃO EM ENFERMAGEM: (DES) ENCONTROS ENTRE A FORMAÇÃO E A ATENÇÃO A SAÚDE

A integração ensino-serviço-comunidade configura-se como instrumento indispensável para a mudança na formação em saúde e para a melhoria da atenção oferecida à população, na medida em que permite a aproximação com as necessidades de vida e saúde dos sujeitos. Este estudo tem como objetivo geral compreender como se configura a integração Ensino-Serviço-Comunidade no curso de graduação em Enfermagem de uma Universidade Pública do Nordeste brasileiro. Para tanto, parte de uma pesquisa qualitativa, com base em um estudo de caso realizado na cidade de Pau dos Ferros/RN, no Curso de enfermagem da Universidade do Estado do Rio Grande do Norte e na Estratégia Saúde da Família do referido município. A pesquisa foi realizada com 28 participantes, entre representantes dos serviços, do ensino e da comunidade. Os dados foram coletados por meio de entrevista semiestruturada e grupo focal. O processamento e interpretação do material empírico se deram com base na análise de conteúdo, modalidade temática, numa perspectiva analítico-crítica, conforme proposto por Minayo. Os resultados apontaram que há diferentes concepções acerca da compreensão sobre integração ensino-serviço-comunidade, com base no entendimento e na vivência dos diversos sujeitos. Evidencia-se também, que as concepções variaram entre sujeitos de um mesmo grupo e de grupos diferentes. Com relação às estratégias utilizadas para a efetivação dessa integração, evidenciou-se que sua concretização parte, na maioria das vezes, da academia e de projetos de intervenção construídos pelos estudantes na realidade dos serviços de saúde e da comunidade. Evidencia-se também, algumas dificuldades apontadas por todos os sujeitos para a efetivação dessa integração, como a distância dos docentes do mundo dos serviços, a organização do estágio, a falta de responsabilização de alguns profissionais que não se sentem coparticipes do processo. Alguns representantes da comunidade apontam ainda, o distanciamento entre a universidade e a comunidade. Foram apontadas também, inúmeras potencialidades com a integração ensino-serviço-comunidade que reverberam na atenção oferecida a população e a mudança na formação e nos serviços de saúde, como algumas atividades desenvolvidas pelos estudantes nas comunidades. Assim sendo, entende-se que embora os desafios se façam presentes durante a materialização dessa integração, é preciso acreditar que a mudança é possível e se faz presente em alguma medida. O compromisso com uma sociedade mais justa e equânime deve ser bandeira de luta de todos os sujeitos imbricados nessa relação de formador e formado, no sentido de fomentar a transformação e a construção de um sistema de saúde resolutivo, justo e eficaz.

Abstract:
The integration between teaching, service and community sets up as an imperative instrument to bring changes in the health care formation and to the improvement of the care delivered to the population, considering that it allows a closer look in the needs of health and life in general of the subject. The main objective of this study is to understand how the integration between teaching, service and community sets up in the undergraduate nursing school of a public university of Northeastern Brazil. In order to accomplish this purpose a qualitative research based in a case report was performed in the city of Pau dos Ferros - RN, in an undergraduate nursing school of the State University of Rio Grande do Norte and in the Family Health Strategy of the same city. The research was performed with 28 participants from community, health care services and teachers. The data were collected through semi structured interview and focus group. The process and interpretation of the data was based on the theoretic background of the content analysis, under an analytic-critical perspective, as proposed by Minayo. The results showed that there are many conceptions regarding the integration between teaching, service and community based on the understanding and experiences of the many participants. It is highlighted as well that those understandings vary from subject to subject from the same group of from different groups. Regarding the strategies applied to achieve such integration, it stood out that its consolidation, for the most part, start form the academy and intervention projects created by the students applied in the reality of the health services and community. Another points worth noting that was pointed out by the subjects that hinder the effectiveness of this integration were the distance of the teachers of the services, the current organization of the internship policy, lack of participation of some professionals that just do not feel like part of the process. Some representatives of the community also pointed out the distance between university and community. Many good outcomes derived of the potential of the integration between teaching, service and community were pointed out as well. They highlighted the improvement in the care delivered to the population and changes in the formation and health services, like some activities developed by the students in the community. Therefore, although there are challenges during the concretion of this integration, it is necessary to believe that changes are possible. The commitment of having a fairer and equal society must be the objective of all subjects involved in this relationship between teacher and student, in order to promote transformation and construction of a decisive, fair and efficient health system.
