HELICOBACTER PYLORI, GRELINA E NUTRIÇÃO: EFEITOS DA PRESENÇA E DA ERRADICAÇÃO DO MICROORGANISMO EM PACIENTES DO SISTEMA ÚNICO DE SAÚDE

A ascensão da obesidade vem se destacando entre as mudanças ocorridas no perfil nutricional mundial, sendo causada pela interação de múltiplos fatores. Nos últimos anos, alguns estudiosos iniciaram pesquisas buscando uma possível associação entre obesidade e infecção gástrica causada pela presença de Helicobacter pylori. Tem sido apontado que esta infecção pode influenciar a ingestão e homeostase calórica através de sua influência no metabolismo de um hormônio chamado grelina, um peptídeo secretado no estômago e implicado no comportamento de ingestão alimentar e regulação do peso corporal. No entanto, o tipo de influência que ocorre ainda é contraditório. Assim, o presente estudo teve por objetivo analisar os efeitos da presença e da erradicação de Helicobacter pylori sobre os níveis de grelina e sobre aspectos nutricionais (estado nutricional e ingestão dietética) em pacientes atendidos pelo Sistema Único de Saúde (SUS). A coleta de dados foi realizada em dois serviços de referência do SUS. Foram captados pacientes que buscavam o serviço de endoscopia e/ou o ambulatório de Gastroenterologia. Os pacientes fizeram pesquisa de presença da bactéria a partir de exames endoscópicos, com biópsias que foram examinadas pelo teste da urease. Assim, foram estratificados em 3 grupos quanto à presença de Helicobacter pylori: positivos, negativos e erradicados. Os níveis séricos de grelina ativa e total foram determinados através do método Elisa. Os pacientes foram avaliados quanto ao estado nutricional, diagnosticado pelo índice de massa corporal e quanto à ingestão dietética de calorias e macronutrientes. Os achados foram confrontados entre os grupos. Os resultados mostraram que a população em estudo tinha baixa renda e escolaridade e ocupação menos especializada. Os pacientes erradicados exibiram uma maior proporção de excesso ponderal, em comparação aos outros grupos (p = 0,011). O consumo de calorias e macronutrientes foi semelhante nos três grupos, com exceção da sacarose, que teve uma ingestão menor no grupo com a bactéria erradicada (p = 0,000). Para os níveis séricos de grelina total e ativa, foram detectados valores mais baixos (variável categórica p = 0,025; variável contínua p = 0,016) e mais altos (variável categórica p = 0,004; variável contínua p = 0,003), respectivamente, para pacientes com a bactéria erradicada. Além disso, os pacientes erradicados apresentam-se em maior proporção nas categorias abaixo do 8 normal para grelina total e normal para grelina ativa. Os níveis de grelina ativa foram associados com o estado nutricional avaliando-se todos os pacientes (p = 0,024) e os valores de grelina total foram menores quando a ingestão de proteína, de gordura polinsaturada, de fibra e de colesterol, foram maiores. Conclui-se que a erradicação da H. pylori é associada com maior presença de excesso ponderal que pode estar sendo acarretado pela alteração dos níveis séricos de grelina detectada nestes pacientes. Estudos subsequentes devem ser desenvolvidos para confirmar e/ou explorar mecanismos de ação envolvidos. O estudo traz uma nova contribuição para este campo de ação, pois este tipo de investigação não integra o protocolo de atendimento aos usuários do SUS. Abstract:
The rise of obesity has standing out among the changes in the global nutritional profile, being caused by the interaction of multiple factors. In the last years, some researchers began research seeking a possible association between obesity and gastric infection caused by the presence of Helicobacter pylori. It has been pointed that infection with Helicobacter pylori can influence ingestion and caloric homeostasis through its influence on the metabolism of a hormone called ghrelin, a peptide secreted in the stomach and implicated in the behavior of food intake and body weight regulation. However, the sort of influence that occurs is still contradictory. Therefore the present study aims to examine the effects of the presence and eradication of Helicobacter pylori over ghrelin levels and nutritional aspects (nutritional status and food intake) in patients attended by SUS. Data collection was performed in two referral centers of the Unified Health System. Patients seeking the service of endoscopy and / or ambulatory of gastroenterology were captured. Patients made research the presence of the bacterium from endoscopy with biopsies were examined by histological method and the urease test. So were stratified into 3 groups according to the presence of Helicobacter pylori: positive, negative and eradicated. Serum levels of total and active ghrelin were determined by ELISA method. Patients were assessed for body mass (body mass index) and dietary intake. The findings of ghrelin levels, anthropometrics, and dietary intake were confronted among groups. The results showed that the study population had low income and education and less specialized occupation. Overweight was found in the three groups, with no statistical difference. However, patients eradicated exhibited a greater proportion of excess weight, compared to the other groups (p = 0,011). Food intake was similar in the three groups considered for almost all nutrients, with the exception of sucrose had a lower intake between the group with eradicated bacteria (p = 0,000). For the serum levels of total and active ghrelin were detected lower values (categorical variable: p = 0,025; continuous variable: p = 0,016) and higher (categorical variable: p = 0,004; continuous variable: p = 0,003), respectively, for patients with H. pylori eradicated. Furthermore, patients eradicated presents in a greater proportion in the categories below normal for total ghrelin and normal for 10 active ghrelin. Active ghrelin levels were associated with nutritional status by evaluating all patients (p = 0.024) and levels of total ghrelin were lower protein intake (in grams), polyunsaturated fat, fiber and cholesterol, where consumption were higher. We conclude that the eradication of H. pylori is associated with increased presence of excess weight that may be being called out by the change of serum ghrelin levels detected in these patients. Subsequent studies should be conducted to confirm and / or explore mechanisms involved. The study brings a new contribution to this field of action, as this type of research is not part of the protocol of care to users of SUS.
