Cultura de segurança do paciente: percepção dos funcionários da farmácia hospitalar

Resumo:
A necessidade do fortalecimento de uma cultura de segurança no nível organizacional é apontada como medida fundamental ao processo de melhoria da segurança do paciente no contexto hospitalar. A metodologia ativa tem permitido a articulação entre a universidade, o serviço e a comunidade, por possibilitar uma leitura e intervenção consistente sobre a realidade, valorizar todos os atores no processo da construção coletiva e seus diferentes conhecimentos, e promover a liberdade no processo de pensar e no trabalho em equipe. O objetivo geral desse estudo é compreender a percepção dos funcionários da farmácia hospitalar sobre a cultura de segurança do paciente no ambiente hospitalar. Utilizou-se como método a pesquisa-ação, na qual o pesquisador norteia o fator de exposição (a intervenção), modificando intencionalmente algum aspecto que tenha como resultado a mudança do estado de saúde de indivíduos. A pesquisa contemplou três fases: antes, durante e após a intervenção, fazendo o movimento necessário para a ação educativa, objeto deste estudo. Foi aplicado um questionário aos participantes, para avaliar o conhecimento prévio sobre segurança do paciente, em seguida, foi realizada uma capacitação, onde a exposição dialogada, a roda de conversa e o circuito foram as estratégias utilizadas, e a terceira etapa, avaliativa, com a reaplicação do questionário e uma avaliação da capacitação realizada. Os resultados evidenciaram que os auxiliares que participaram da pesquisa conseguiam identificar os principais elementos associados à cultura de segurança do paciente, no entanto, necessitam de treinamentos e capacitações para melhor desenvolver suas atividades, com base nos conceitos norteadores desse assunto. Por ser recente a discussão sobre cultura de segurança em instituições de saúde no Brasil, há poucos trabalhos publicados sobre a temática envolvendo o setor de farmácia, motivo pelo qual boa parte das referências apresentadas nessa pesquisa está relacionada à área de enfermagem. A realização dessa pesquisa evidenciou estratégias identificadas por profissionais de saúde para construção da cultura de segurança do paciente, no entanto, também gerou inquietude, visto que, a instituição alvo desse estudo ainda não sistematizou ações voltadas para a segurança do paciente. Abstract:
The need to strengthen a safety culture at the organizational level is seen as a key measure to improve patient safety process in the hospital context. The active methodology has allowed the relationship between the university, the service and the community by allowing a reading and consistent intervention on reality, valuing all stakeholders in the process of collective construction and their different knowledge and promote freedom in the process of thinking and teamwork. The general objective of this study is to understand the perception of the hospital pharmacy staff about patient safety culture in the hospital environment. It was used as a method action research, in which the researcher guides the exposure factor (the intervention), intentionally modifying some aspect that results in the change of the health status of individuals. The research included three stages: before, during and after the intervention, making the move necessary for the educational action, object of this study. A survey participant to assess the prior knowledge of patient safety has been applied, then one training was performed, where the dialogical exposure, the conversation wheel and the circuit were utilized, and the third step strategy was evaluative, with reapplication of the questionnaire and an evaluation of the training carried out. The results showed that the assistants who participated in the survey could identify the main elements associated with patient safety culture, however, require training and capacity building to better develop their activities, based on the guiding concepts of this subject. Being recent discussion on safety culture in health institutions in Brazil, there are few published studies on the subject, involving the pharmaceutical industry, which is why, most of the references presented in this research are related to nursing. The realization of this research showed strategies identified by health professionals for the construction of the patient safety culture, however, has generated concern, since the target institution of study, not yet systematized actions for patient safety.
