Resumo:

A descentraliza��o e a regionaliza��o s�o o ponto de partida para a reorganiza��o da
rede de aten��o � sa�de. Quest�o norteadora da pesquisa: como tem se dado a
atua��o da CIR na governan�a da reestrutura��o das Redes de Aten��o � Sa�de
(RAS) na Primeira Regi�o de Sa�de do Estado do Cear�? Objetivo geral: analisar a
atua��o da Comiss�o Intergestores Regional (CIR) na governan�a do processo de
reestrutura��o das Redes de Aten��o � Sa�de na Primeira Regi�o de Sa�de do
Cear�. Objetivos espec�ficos: caracterizar as redes locais de sa�de dos sistemas
municipais da Primeira Regi�o de Sa�de do Cear�; identificar as contribui��es da
Comiss�o Intergestores Regional (CIRs) da 1� CRES na governan�a regional para a
estrutura��o da RAS na Primeira Regi�o de Sa�de do Cear�. T�cnicas de produ��o
dos dados: entrevista, observa��o direta e an�lise documental. Sujeitos: gestores da
sa�de, secret�rios municipais e coordenadores regionais de sa�de dos munic�pios
Aquiraz, Eus�bio, Fortaleza e Itaitinga. Campo da pesquisa: Secretarias Municipais
de Sa�de e Coordenadorias Regionais de Sa�de; reuni�es da CIR no per�odo de 2016
a 2017. Quanto � governan�a na sa�de, os interlocutores afirmaram que os gestores
municipais t�m grande influ�ncia na defini��o das pol�ticas regionais de sa�de atrav�s
dos canais de governan�a (CIB/CIR) quando pactuam e negociam estrat�gias e
prioridades entre Estado e munic�pios, bem como fortalecem a gest�o regional da
sa�de. O Minist�rio da Sa�de segue sendo o grande protagonista da organiza��o do
SUS como detentor do poder regulat�rio com suas normativas e portarias. A esfera
nacional � centralizadora do poder, restando �s inst�ncias subnacionais a governan�a
que operacionaliza a pol�tica. O que se poderia chamar de �operacional� ou
�procedimental�, uma vez que os secret�rios de sa�de n�o decidem sobre a pol�tica,
mas apenas fazem acordo sobre procedimentos que j� v�m determinado desde o
n�vel central para os munic�pios. Seus poderes est�o limitados a pactua��es e
acordos sobre como dar encaminhamento a decis�es pol�ticas previamente
determinadas em inst�ncias superiores.

Abstract:

Decentralization and regionalization are the starting point for the reorganization of the
health care network. Guiding question of the research: how has CIR been acting in the
governance of the Restructuring of Health Care Networks (RAS) in the First Region of
Health of the State of Cear�? General objective: to analyze the performance of the
Regional Interagency Commission (CIR) in the governance of the process of
restructuring the Health Care Networks in the First Health Region of Cear�. Specific
objectives: characterize the local health networks of the municipal systems of the First
Health Region of Cear�; to identify the contributions of the Regional Interagency
Committee (CIRs) of the 1st CRES in the regional governance for structuring SAR in
the First Health Region of Cear�. Techniques of data production: interview, direct
observation and documentary analysis. Subjects: health managers, municipal
secretaries and regional health coordinators of the municipalities Aquiraz, Eus�bio,
Fortaleza and Itaitinga. Field of research: Municipal Health Secretariats and Regional
Health Coordination; meetings of the CIR between 2016 and 2017. Concerning health
governance, the interlocutors affirmed that municipal managers have a great influence
on the definition of regional health policies through the channels of governance (CIB /
CIR) when they negotiate strategies and priorities between the state and
municipalities, as well as strengthen regional health management. The Ministry of
Health continues to be the main protagonist of the SUS organization as the holder of
the regulatory power with its regulations and ordinances. The national sphere is
centralizing power, leaving to the subnational instances the governance that
operationalizes the policy. What could be called "operational" or "procedural", since
health secretaries do not decide on the policy, but only agree on procedures that have
already determined from the central level to the municipalities. Its powers are limited
to agreements and agreements on how to give direction to previously determined
political decisions in higher instances.
