SUBJETIVIDADES À DERIVA: DIÁLOGOS POSSÍVEIS COM OS
TRABALHADORES DE SAÚDE MENTAL E O CONJUNTO DE PRÁTICAS

Resumo:
Este trabalho abordou a temática da subjetividade em trabalhadores de saúde
mental, tendo por objeto os trabalhadores de saúde mental no conjunto de práticas
teóricas, técnicas e políticas. Esse objeto teve como motivação a experiência da
pesquisadora na Rede de Atenção Psicossocial (RAPS) de Fortaleza por mais de
uma década, o que suscitou como principal questionamento: será antimanicomial o
conjunto de práticas dos trabalhadores dos Centros de Atenção Psicossocial
(CAPS)? Para dialogar com esta questão, a pesquisa teve como objetivo geral:
compreender as relações entre a subjetividade e as práticas dos trabalhadores de
saúde mental na atenção psicossocial territorial, e como específicos: analisar as
matrizes políticas, técnicas e teóricas oferecidas aos trabalhadores pelo Movimento
Brasileiro de Reforma Psiquiátrica (MBRP) e pela RAPS; analisar as práticas dos
trabalhadores na concretude de um caso; comparar as matrizes e as práticas
concretas na perspectiva dos trabalhadores do caso; sistematizar as identificações,
lacunas e contradições entre as matrizes políticas, técnicas e teóricas e as práticas
analisadas, comparadas e criticadas. Apresentaram-se as seguintes perguntas
norteadoras: a) qual sua opinião sobre os princípios, as diretrizes e formas de
trabalho dos hospitais psiquiátricos clássicos e da proposta dos CAPS? b) em sua
opinião, quais identificações, lacunas e contradições ocorrem entre o que se propõe
e o que se faz nos CAPS? Estão presentes nesse trabalho as categorias - psiquiatria
clássica, atenção psicossocial territorial, saúde mental, trabalho/trabalhador em
saúde mental e subjetividade. Foram utilizados autores importantes: do MRP no
mundo, Basaglia, Rotelli, Desviat, e no Brasil, Amarante, Bezerra Júnior, Delgado,
Passos, Sampaio; da filosofia Foucault, Deleuze, Guattari, dentre outros. O
delineamento deste projeto se constituiu como um estudo de caso, qualitativo,
analítico-crítico, que se operacionalizou etnograficamente, apoiando-se em
observação participante, entrevista em profundidade, grupo-experimentação, diário
de campo e fotografia. O caso escolhido foi o CAPS Geral da Coordenadoria
Regional de Saúde (CRS) III de Fortaleza, devido à representatividade como o
primeiro CAPS deste município. Os sujeitos foram os trabalhadores deste serviço,
após aplicação dos critérios de inclusão e exclusão. O método utilizado de análise
foi inspirado na análise temática de Minayo. Os achados da pesquisa, após análise e
discussão, foram apresentados em uma síntese em três “nós”: a precarização na
forma de recrutamento dos trabalhadores de saúde mental; a carência de um
espaço dialógico, teórico e vivencial que possibilite aos trabalhadores se fortalecer
enquanto um coletivo singular; e a problematização do cuidado ofertado neste
serviço que ainda se apresenta como médico centrado. Portanto, a pesquisa
vislumbrou, como pressuposto, que a dimensão da subjetividade do trabalhador de
saúde mental poderá construir práticas asilares ou antimanicomiais nas políticas
públicas de saúde mental, mesmo quando constituídas a partir de perspectivas
antimanicomiais.

Abstract:
This work addressed the issue of subjectivity in mental health workers, focusing on
mental health workers in the set of theoretical, technical, and political practices. The
researcher‟s experience in the Psychosocial Care Network (RAPS) of Fortaleza-CE,
Brazil, for more than a decade motivated this subject and raised the main question: Is
the set of practices of Psychosocial Care Centers (CAPS) workers anti-asylum? To
approach this question, the general objective of this study was to understand the
relationship between subjectivity and practices of mental health workers in territorial
psychosocial care. While the specific objectives were to analyze the political,
technical, and theoretical matrices offered to workers by the Brazilian Psychiatric
Reform Movement (BPRM) and RAPS; analyze the practices of workers in individual
cases; compare matrices and concrete practices from the perspective of those
working on the case; systematize the identifications, gaps, and contradictions
between the political, technical, and theoretical matrices and the analyzed,
compared, and criticized practices. Thus, the following guiding questions were
presented: a) What is your opinion about the principles, guidelines, and working
methods of classic psychiatric hospitals and the CAPS proposal? b) In your opinion,
what are the identifications, gaps, and contradictions between what is proposed and
what is done in the CAPS? This work comprises the categories: classical psychiatry,
territorial psychosocial care, mental health, mental health work/worker, and
subjectivity. We used important authors from the Psychiatric Reform Movement in the
world: Basaglia, Rotelli, Desviat; and in Brazil: Amarante, Bezerra Júnior, Delgado,
Passos, Sampaio; and from philosophy: Foucault, Deleuze, Guattari, among others.
This project consisted of a qualitative, critical and analytical case study
ethnographically operationalized, supported by participant observation, in-depth
interview, group experimentation, field diary, and photography. The case selected
was the CAPS General of the third Regional Health Coordination (CRS) of Fortaleza,
given its relevance as the first CAPS of this municipality. The subjects were the
workers of this service, after applying the inclusion and exclusion criteria. The
method of analysis used was based on the thematic analysis proposed by Minayo.
After analysis and discussion, a synthesis of the findings was presented in three
“nodes”: the precarious form of recruiting mental health workers; the lack of a
dialogical, theoretical, and experiential space that enables workers to strengthen
themselves as a singular collective; and the problematization of the care provided in
this service that can still be described as doctor-centered. Therefore, this research
verified, as expected, that the subjectivity dimension of mental health workers may
construct asylum or anti-asylum practices in public mental health policies, even when
constituted from anti-asylum perspectives.
