INTERAÇOES ENTRE ESTRESSE OXIDATIVO E PERFIL NUTRICIONAL EM
PACIENTES PRÉ-TRANSPLANTE DE CÉLULAS TRONCO HEMATOPOIÉTICAS

RESUMO No transplante de células tronco hematopoiéticas (TCTH) o estado nutricional é um fator determinante do prognóstico, estando relacionado com o risco de doença do enxerto versus hospedeiro. Já o estresse oxidativo parece estar relacionado ao tempo de “pega” do enxerto. O desequilíbrio oxidativo pode ser atenuado com a ingestão de vitaminas antioxidantes e com a manutenção de um bom estado nutricional. Entretanto, a ingestão alimentar pode ser abalada pelos efeitos adversos dos medicamentos usados antes do transplante. O objetivo do presente trabalho é avaliar as interações entre estado nutricional, consumo alimentar e estresse oxidativo em pacientes pré-TCTH. O estudo do tipo transversal foi realizado com pacientes do ambulatório de hematologia do Hospital Universitário Walter Cantídeo. Coletou-se dados demográficos, dados clínicos (doença e tipo de transplante), avaliação subjetiva global produzida pelo próprio paciente e indicadores antropométricos como circunferência da cintura, índice de conicidade, percentual de gordura corporal, força muscular e presença de sarcopenia. Realizou-se coleta de 4 mL de sangue de cada indivíduo para determinação do estresse oxidativo. O estresse oxidativo foi analisado por níveis de substâncias reativas ao ácido tiobarbitúrico. Além disso, realizou-se dois recordatórios alimentares de 24h para estimativa de consumo habitual de energia, macronutrientes, micronutrientes e fibras. Os dados foram expressos em frequências, percentuais, média, desvio-padrão e as análises estatísticas foram realizadas através do softtware SPSS (versão 19), o p<0,05 foi definido como estatíticamente significante. O projeto de pesquisa foi submetido ao Comitê de Ética em Pesquisa do referido hospital. A população do presente estudo foi constituída de 72 pacientes com indicação de TCTH, sendo a maioria do sexo masculino (56,9%) e idade média de 48,93 (±14,5) anos. A patologia prevalente foi mieloma múltiplo (51,4%). Conforme Índice de Massa Corpórea, 55,6% dos pacientes encontravam-se com excesso de peso. De acordo com a circunferência da cintura, índice de conicidade e percentual de gordura corporal houve prevalência para risco de doença cardiovascular. Sarcopenia esteve associada ao risco de doença cardiovascular pela razão cintura quadril (p=0,021); depleção da força muscular de acordo com a dinamometria (p<0,001); alteração da ingestão alimentar (p=0,23), capacidade funcional (p=0,048) e classificação do estado nutricional (p=0,044), conforme ASG-PPP. Dentre os avaliados, foram obtidos dados referentes à ingestão alimentar de 63 participantes. Desses, 96,8% apresentaram consumo adequado de proteína e 87,3% consumo inadequado de calorias. O consumo de vitamina B6 e manganês associou-se à presença de sarcopenia (p=0,022). De acordo com a avaliação do estresse oxidativo, 66,7% dos avaliados apresentaram estresse oxidativo baixo, não houve associação significativa entre estresse oxidativo e as variáveis estudadas. Concluiu-se que pacientes pré-TCTH apresentaram baixo estresse oxidativo, porém com depleção muscular importante (sarcopenia), ingestão inadequada de nutrientes e risco de DCV. Abstract:
ABSTRACT In hematopoietic stem cell transplantation (HSCT) the nutritional status is a determinant of the prognosis and is related to the risk of graft versus host disease. Already the oxidative stress seems to be related to the time of grafting. Oxidative imbalance can be attenuated with the ingestion of antioxidant vitamins and maintaining a good nutritional status. However, food intake may be affected by the adverse effects of medications used before transplantation. The objective of the present study is to evaluate the interactions between nutritional status, dietary intake and oxidative stress in pre-HSCT patients. The cross-sectional study was performed with patients from the hematology clinic of the Walter Cantídeo University Hospital. Demographic data, clinical data (disease and type of transplantation), global subjective evaluation produced by the patient and anthropometric indicators such as waist circumference, conicity index, percentage of body fat, muscle strength and presence of sarcopenia were collected. Four mL of blood was collected from each individual to determine oxidative stress. Oxidative stress was analyzed by levels of thiobarbituric acid reactive substances. In addition, two 24-hour food reminders were used to estimate the usual energy consumption, macronutrients, micronutrients and fibers. Data were expressed as frequencies, percentages, mean, standard deviation and statistical analyzes were performed using SPSS software (version 19), p <0.05 was defined as statistically significant. The research project was submitted to the Research Ethics Committee of the referred hospital. The population of the present study consisted of 72 patients with an indication for HSCT, being the majority male (56.9%) and mean age of 48.93 (± 14.5) years. The prevalent pathology was multiple myeloma (51.4%). According to the Body Mass Index, 55.6% of the patients were overweight. According to the waist circumference, conicity index and percentage of body fat there was prevalence for cardiovascular disease risk. Sarcopenia was associated with the risk of cardiovascular disease by waist ratio (p = 0.021); the depletion of muscle strength according to dynamometry (p <0.001); (p = 0.23), functional capacity (p = 0.048) and nutritional status (p = 0.044), according to ASG-PPP. Among those evaluated, data on food intake were obtained from 63 participants, of whom 96.8% had adequate protein intake and 87.3% had inadequate calorie intake. The consumption of vitamin B6 and manganese were associated with the presence of sarcopenia (p = 0.022). According to the evaluation of oxidative stress, 66.7% of the evaluated ones presented low oxidative stress, there was no significant association between oxidative stress and the studied variables. It was concluded that pre-HSCT patients presented low oxidative stress, but with significant muscular depletion (sarcopenia), inadequate nutrient intake and risk of CVD.
