RESUMO
Na presente pesquisa, busca-se compreender, por meio da reflexão e intervenção sobre o planejamento em saúde, as relações entre Políticas de Saúde e Epidemiologia no enfrentamento de uma situação concreta de crise assistencial e financeira no sistema de saúde de Iguatu, município localizado no interior cearense. Trata-se de uma pesquisa-ação participante que enfoca o planejamento municipal em saúde como objeto de reflexão e intervenção. A reflexão parte do consumo das metodologias de planejamento pelos atores em uma situação de crise de um sistema municipal de saúde. A intervenção se baseia nas metodologias de planejamento existentes e consumidas pelos atores, portanto em suas transformações de teorias em práticas, para elaboração do Plano Municipal de Saúde 2014-2017, da Programação Anual de Saúde de 2014 e de 2015 e do Relatório Anual de Gestão de 2014. O objeto de pesquisa é uma prática constituída socialmente, portanto sua interpretação e transformação também são considerados um processo social. O desdobramento desses pressupostos para operacionalização da pesquisa foi realizado por meio de uma crítica às relações entre tática, estratégia e poder no planejamento em saúde. O engajamento dos participantes para transformar o sistema de saúde e a situação sociossanitária de Iguatu por meio da construção dos dispositivos de planejamento, do fortalecimento da descentralização e democratização da gestão e da instituição de novas práticas de cuidado em saúde foi satisfatória. O uso das esferas públicas para mediar ação e reflexão por meio do diálogo e do consenso foi pontuado por dificuldades operacionais, porém essa alternativa mostrou-se uma proposta ética e politicamente adequada aos pressupostos emancipatórios defendidos pelas correntes teóricas a que se recorreu como fundamento dessa Tese. A chefia Guarani emergiu como desafio ético e político para a transformação participativa e democrática das práticas de atenção e gestão à saúde em Iguatu. Seus resultados foram moderadamente satisfatórios nos serviços de saúde mental, mas apenas neles. Não se observou modificação expressiva do processo de trabalho e gestão em saúde nos demais setores e serviços de Iguatu. Por outro lado, as discussões no Conselho Municipal de Saúde tornaram-se mais densas e críticas após a experiência da elaboração dos dispositivos do planejamento em saúde. Por se tratar de um estudo de caso, essa pesquisa apresenta pequeno potencial de generalização no seu todo.

Porém, algumas de suas partes podem ser consideradas como paradigmáticas para outros municípios de médio e grande porte do interior cearense, como a discussão sobre a precarização do processo de trabalho e a incorporação de novas tecnologias em saúde.

Abstract:
ABSTRACT
This research aims to understand, by pondering upon and intervening in health planning, the relation between Health Policy and Epidemiology dealing with an actual care and financial crisis situation in the health system of Iguatu, a city in the interior of State of Ceará – Brazil. It is a participatory action research that focuses on the municipal health planning as an object of pondering and intervention. The pondering starts upon the planning methodology used by the actors in a crisis situation of a municipal health system. The intervention is based on the existing planning methodology that were used by the actors, therefore to transform their theories into practices for the elaboration of the 2014-2017 Municipal Health Planning, the 2014 and 2015 Annual Health Projects and the 2014 Annual Administration Report. The object of research is a socially constructed practice, therefore its interpretation and transformation are also taken as a social process. The premises to the implementation of the research was made by a critique of the relation between tactics, strategy and power in the health planning. The commitment of the participants to the transformation of the health system and the sociosanitary situation of Iguatu by creating planning devices, strengthening the decentralization and democratization of the administration and implanting new health care practices was considered satisfactory. The use of public spheres to mediate action and reflection using the dialog and the consensus was stained by operational difficulties, but this alternative was shown to be a proposal that was ethically and politically fit to the emancipating premises sustained by the theoretical framework used as a foundation of this Thesis. The Guarani Leadership emerged as an ethical and political challenge for the democratic and participatory changes in the health care and administration of Iguatu. Its results were moderately satisfactory on the mental health services, but only on these. No expressive modification was observed on the health work and administration processes of the other areas and services of Iguatu. On the other hand, the debates of the Municipal Health Council became more thorough and critical after the experience of creating the health planning devices. Being a case study, this research poses a small generalization potential in its whole. Nevertheless, some of its parts might be considered as a paradigm to other medium and big cities of the interior of State of

Ceará such as the precarious work process and the incorporation of new health technologies.
