POSSIBILIDADES E IMPOSSIBILIDADES NO ALEITAMENTO MATERNO DO BEBÊ COM CARDIOPATIA CONGÊNITA: A ESCUTA DO DISCURSO MATERNO

RESUMO
Esta é uma pesquisa de desenvolvimento descritivo, com abordagem qualitativa, que teve como objetivo compreender a vivência das mães no aleitamento materno de seus bebês com cardiopatia congênita. A pesquisa foi realizada na unidade de cardiopediatria do Hospital Dr. Carlos Alberto Studart Gomes (Hospital de Messejana) em Fortaleza-Ceará, instituição de reconhecimento nacional e internacional no tratamento de pacientes com cardiopatia congênita. A coleta de informações ocorreu nos meses de fevereiro a abril de 2016, com a participação de nove mães de bebês diagnosticados com cardiopatia congênita, sendo incluídas mães adolescentes e adultas, com idade de ≥ 10 anos a < 60 anos. Após esclarecimento e assinatura dos termos de consentimento e assentimento, as mães responderam questões sobre dados sociodemográficos, seguidos de uma entrevista semiestruturada, abordando a experiência vivida no aleitamento de seu bebê com cardiopatia. As entrevistas foram gravadas com consentimento, respeitando os aspectos éticos, e suas falas, posteriormente, transcritas na íntegra para análise. Por meio de processo reducional, seguindo as fases propostas da análise temática de Minayo, foram obtidos 242 recortes expressivos, acomodados em 27 unidades de significado e organizadas em duas categorias: ”O saber materno sobre a doença e a internação do seu bebê” e “A mãe ante a possibilidade e impossibilidade de amamentar seu bebê com cardiopatia congênita”. As vivências maternas apontaram fatores que interferem no aleitamento materno, tais como o estado emocional da mãe ante a cardiopatia do filho, a atitude de alguns profissionais pouco experientes em cardiopediatria que, de imediato, inabilitam o aleitamento materno e a ausência, estrutural e organizacional, de serviço que favoreça a prática. Com base nos discursos maternos foi elaborado um livro de colorir, a ser utilizado com esse público, no intuito de reduzir a ansiedade e amenizar o estado emocional fragilizado da mãe diante da doença e internação do bebê, porquanto que estados de estresse interferem na lactação e prejudicam o aleitamento materno.
This is a descriptive development research, with qualitative approach that aimed to understand the experience of mothers in breastfeeding their babies with congenital heart disease. The survey was conducted in Cardiopediatrics unit of the Hospital Dr. Carlos Alberto Gomes Studart (Messejana Hospital) in Fortaleza, Ceará, national and international recognition of the institution in the treatment of patients with congenital heart disease. Data collection took place from February to April 2016, with the participation of nine mothers of babies diagnosed with congenital heart disease, adolescent and adult mothers were included, aged ≥ 10 years <60 years. After clarification and signing the terms of consent and assent, the mothers answered questions about sociodemographic data, followed by a semi-structured interview, addressing the experience in feeding your baby with heart disease. The interviews were recorded with consent, respecting the ethical, and their speeches later transcribed for analysis. Through reductional process, following the steps proposed the theme of Minayo analysis, 242 significant cuts, accommodated in 27 units of meaning and organized into two categories were obtained: "Knowledge mother about the illness and hospitalization of your baby" and " The mother at the possibility and impossibility of breastfeeding your baby with congenital heart disease. " Maternal experiences pointed to factors that interfere with breastfeeding, such as the mother's emotional state before the disease's son, the attitude of some inexperienced professionals Cardiopediatrics that immediately disqualify breastfeeding and absence, structural and organizational, service that encourages the practice. Based on maternal discourse has produced a coloring book to be used with this audience in order to reduce anxiety and soften the fragile emotional state of the mother before the illness and hospitalization of the baby, because that stress states interfere with lactation and undermine breastfeeding.
