DESENVOLVIMENTO E VALIDAÇÃO DE INSTRUMENTO DE AVALIAÇÃO DA ADESÃO AO TRATAMENTO DA INSUFICIÊNCIA CARDÍACA

RESUMO
A Insuficiência Cardíaca (IC) é a via final comuns das doenças que acometem o coração, sendo um dos maiores desafios atuais na área da saúde. A relevância de estudar essa doença se deve aos aspectos epidemiológicos e clínicos envolvidos e por se tratar de um grave problema de saúde pública pela sua magnitude e transcendência, sobretudo na ausência de adesão ao tratamento farmacológico e não-farmacológico. Os instrumentos atuais que avaliam adesão ao tratamento da IC não dão conta de apreender o fenômeno em sua integralidade. Assim, foi objetivo do presente estudo desenvolver um instrumento de avaliação da adesão ao tratamento da insuficiência cardíaca, e sua validação. Trata-se de um estudo metodológico, com roteiro baseado no PROMIS (Patient Reported Out come Measurement Information System). A construção do instrumento seguiu as etapas: 1) revisão integrativa para elucidar o construto da adesão ao tratamento da IC; 2) grupo focal com os acometidos pela doença, em busca de melhor compreensão de seus domínios da IC; 3) construção do instrumento de medida a partir dos instrumentos já existentes; 4) validação de conteúdo com os juízes especialistas da área e 5) avaliação da compreensão do instrumento por meio da entrevista cognitiva com os respondentes. O estudo foi aprovado pelo Comitê de Ética do Hospital de Messejana Dr. Carlos Alberto Studart Gomes no processo n° 64053617.0.0000.5039. Os resultados são oriundos de duas revisões integrativas, quatro grupos focais com a participação total de 24 pessoas, validação de face e conteúdo com sete especialistas e entrevista cognitiva com nove pacientes. Tomou-se como base o documento de adesão ao tratamento de doenças crônicas da Organização Mundial de Saúde (OMS, 2003). Foram encontrados 20 instrumentos e o banco de itens final continha os domínios: paciente, doença/tratamento, sistema de saúde e ambiente. Após a consolidação dos questionários, foram incluídos 38 itens que abordavam domínios da adesão ao tratamento da IC. Dos 38 itens do instrumento final, 37 permaneceram na versão final. O índice de Validade de Conteúdo foi 0,98 e o coeficiente alfa de Cronbach do instrumento foi de 0,92. Por fim, dispõe-se de uma tecnologia para mensurar a adesão ao tratamento da insuficiência cardíaca, validada teoricamente. Pretende-se em um momento posterior realizar a validação clínica por meio da Teoria da Resposta ao Item.
Heart Failure is the common final pathway of heart disease, one of the greatest challenges in the health field today. The importance of studying this disease is due to the epidemiological and clinical aspects involved and because it is a serious public health problem due to its magnitude and transcendence, especially in the absence of adherence to pharmacological and non-pharmacological treatment. The current instruments that evaluate adherence to the treatment of HF do not account for apprehending the phenomenon in its entirety. Thus, it was the objective of the present study to develop an instrument to evaluate adherence to the treatment of heart failure, and its validation. It is a methodological study, based on the PROMIS (Patient Reported Outcome Measurement Information System). The construction of the instrument followed the steps: 1) integrative review to elucidate the construct of adherence to the treatment of HF; 2) focal group with those affected by the disease, in search of a better understanding of the domains of HF; 3) construction of the measuring instrument from existing instruments; 4) content validation with area experts and 5) evaluation of the understanding of the instrument through the cognitive interview with the respondents. The Ethics Committee of the Hospital de Messejana Dr. Carlos Alberto Studart Gomes under case 64053617.0.0000.5039 approved the study. The results come from two integrative reviews, three focus groups with a total participation of 24 people, validation of face and content with seven specialists and cognitive interview with nine patients. The document on adherence to the treatment of chronic diseases of the World Health Organization (WHO, 2003) was used as a basis. We found 20 instruments and the final item bank contained the domains: patient, disease / treatment, health system and environment. After consolidation of the questionnaires, we included 38 items that addressed domains of adherence to HF treatment. Of the 38 items of the final instrument, 37 remained in the final version. The Content Validity index was 0.98 and the instrument's Cronbach alpha coefficient was 0.92. Finally, there is a technology to measure adherence to the treatment of heart failure, validated theoretically. It is intended at a later time to carry out the clinical validation through the Theory of Response to the Item
