ELABORAÇÃO E VALIDAÇÃO DE CADERNETA DE ORIENTAÇÃO EM SAÚDE PARA FAMILIARES DE CRIANÇAS COM DOENÇA FALCIFORME

RESUMO
As doenças falciformes possuem particularidades fisiológicas e clínicas, havendo situações de alerta quanto à possível crise ou complicação grave. Assim, esta tese teve como objetivo desenvolver uma tecnologia (caderneta) de acompanhamento e orientação em saúde sobre a doença falciforme para familiares de crianças com essa enfermidade, e sua validação. Método: estudo misto, em três etapas: a primeira foi uma pesquisa qualitativa, realizada no hospital de referência pediátrica no Ceará, com 12 familiares de crianças com doença falciforme, visando identificar as principais necessidades de orientação em saúde; a segunda etapa foi metodológica, de elaboração e validação interna da caderneta, com avaliação de nove juízes especialistas, oito técnicos, sete de comunicação (validação de conteúdo e aparência) e 12 representantes do público alvo (análise semântica e de aparência); a terceira etapa consistiu-se em um quase-experimento (validação clínica), visando testar a aplicabilidade da caderneta, com a participação de 20 familiares, envolvendo ações pré-teste e pós-teste, onde cada indivíduo foi seu próprio controle, e a análise dos dados foi realizada no programa SPSS 20 (utilizou-se os testes: T de Student; Kruskal-Wallis; Correlação de postos de Kendall; Qui-Quadrado de MCnemar). A pesquisa foi aprovada pelo Comitê de Ética da Universidade Estadual do Ceará (nº 1.955.727) e do referido hospital pediátrico (nº1.994.879). Resultados: a primeira etapa revelou que as orientações acerca da doença falciforme eram incipientes e os familiares revelaram o anseio de serem melhor esclarecidos. Na segunda etapa, a avaliação dos juízes técnicos e especialistas alcançou Índice de Validade de Conteúdo global de 0,93, caracterizando a caderneta como de boa qualidade; a avaliação por tópicos individuais, revelou que apenas dois não estavam adequados e os demais tiveram índices entre 0,82 e 1. Os juízes de comunicação avaliaram todos os itens como Superior. Após os ajustes, a caderneta sofreu análise semântica pelos familiares, atingindo 100% de concordância em todos os itens. A terceira etapa evidenciou aumento, estatisticamente significante, entre a média das pontuações no pós-teste, quando comparado ao pré-teste. Concluiu-se que a Caderneta “Cuidando de uma criança com doença falciforme”, após validada quanto ao conteúdo e a aparência, e análise semântica, contribuiu para o aumento do conhecimento de familiares de crianças com essa doença.

ABSTRACT
Sickle diseases present physiological and clinical characteristics, and there are alert situations regarding the possible crisis or serious complication. Thus, this thesis aimed to develop a technology (leaflet) of health monitoring and guidance on sickle cell disease in relatives of children with this disease, and its validation. Method: a three-step, mixed study: the first was a qualitative study, carried out in a pediatric referral hospital in Ceará, with 12 relatives of children with sickle cell disease, with the objective of identifying the main health orientation needs; the second stage was methodological, internal preparation and validation of the book, with evaluation of nine expert judges, eight technicians, seven communication (content validation and appearance) and 12 representatives of the target public (semantic analysis and appearance); the third stage consisted of a quasi-experiment (clinical validation), with the objective of testing the applicability of the book, with the participation of 20 family members, involving pre-test and post-test actions, where each individual was their own control , and Data analysis was performed in the SPSS 20 program (Student's t-test, Kruskal-Wallis, Kendall test correlation, and MCnemar chi-square test). The study was approved by the Research Ethics Committee of the State University of Ceará (nº 1,955,727) and of the said pediatric hospital (nº 1,994,879). Results: The first step revealed that the guidelines for sickle cell disease were incipient and family members expressed a desire to be better informed. In the second stage, the evaluation of technical judges and experts reached a Global Content Validity Index of 0.93, characterizing the book as good quality; the evaluation by individual topics revealed that only two were not adequate and the others had indices between 0.82 and 1. The judges of the communication evaluated all items as Superior. After the adjustments, the book was analyzed semantically by family members, reaching 100% agreement on all items. The third step presented a statistically significant increase among the post-test scores, when compared to the pre-test. It was concluded that the booklet "Caring for a child with sickle cell disease", after being validated in terms of content and appearance and semantic analysis, contributed to increase the knowledge of the relatives of children with this disease.
