Qualidade da assistência pré-natal e desfechos das gestações de mulheres indígenas do Ceará

Resumo:
Estudar variáveis relacionadas ao perfil de grupos populacionais específicos é
requisito básico para que as ações de saúde sejam efetivas. A população
indígena é considerada grupo vulnerável aos agravos à saúde pelas condições
de vida a que está sujeita e há uma escassez de pesquisas que contemple
povos indígenas do Nordeste brasileiro. O objetivo desse trabalho foi avaliar a
qualidade da assistência pré-natal e os desfechos das gestações das mulheres
indígenas das áreas trabalhadas pelas equipes de saúde do Distrito Sanitário
Especial Indígena do Ceará no ano de 2009, além de compará-los com os
achados de um grupo de mães não indígenas que tiveram seus filhos na mesma
época e que moram nas mesmas cidades que as mães indígenas. Para tal
finalidade realizou-se estudo descritivo do tipo caso-controle, no qual as
indígenas foram consideradas caso e as não indígenas, controle. O estudo
aconteceu em seis municípios cearenses onde existem equipes de saúde da
Fundação Nacional de Saúde e considerou uma amostra de 165 mães indígenas
e 148 não indígenas. Para a coleta de dados realizaram-se entrevistas semiestruradas, aplicadas no período de janeiro a outubro de 2009. Informações
também foram obtidas através de cartões da gestante e declarações de
nascidos vivos e de óbitos. Os dados foram analisados no software Stata v. 7.
As proporções foram comparadas através do Teste do Qui-Quadrado e Exato de
Fisher. Consideraram-se significativamente diferentes as probabilidades
menores que 0,05. A pesquisa mostrou que o perfil sociodemográfico das
indígenas e não indígenas foi bastante semelhante. A média de idade das
indígenas foi de 24 anos (± 6,5) e das não indígenas 25 anos (± 6,5), maioria
das indígenas (73,6%) e não indígenas (70,1%) com união estável (valor-p=
0,487) e com média de 7 (indígenas) e 8 (não indígenas) anos de estudo, sem
ocupação formal. As mães indígenas (82,8%) tiveram mais acesso à água
encanada que as não indígenas (70,8%), valor-p = 0,012. Observou-se que as
mães indígenas iniciaram o pré-natal mais tardiamente que as não indígenas
(52,5% versus 38,6%, p = 0,015). O nível 1 de Coutinho classificou o pré-natal
das indígenas (63,3%) e não indígenas (50,3%) como intermediário, mas com
2,5% das indígenas e 1,4% das não indígenas com pré-natal inadequado (valorp = 0,041). De acordo com os resultados, conclui-se que as características
sociodemográficas e a qualidade da assistência pré-natal prestada são muito
semelhantes para os dois grupos, com diferença apenas quanto ao início e
número de consultas pré-natal que teve piores resultados entre as mães
indígenas. Não foram observadas diferenças de significância estatística nos
desfechos maternos e perinatais. De uma maneira geral, podemos dizer que a
qualidade da assistência pré-natal e os desfechos obstétricos e perinatais são
muito semelhantes entre a população indígena e as usuárias do SUS não indígenas no Estado do Ceará.

Abstract:
Studying variables related to the profile of specific population groups is a basic
requirement in order to health measures to be effective. The indigenous population is
considered to be a group vulnerable to health problems because of the living conditions
they are subjected to, as well as there being scarce research that takes the indigenous
people of the Brazilian Northeast into account. The objective of this study was to
evaluate the quality of prenatal care and the final outcome of the pregnancies of
indigenous women from the areas covered by the health teams of the Indigenous
Special Sanitary District of Ceará (DSEI) in 2009, and furthermore comparing them with
the findings of a group of non-indigenous mothers who gave birth to their children at the
same period and who live in the same cities as the indigenous mothers. To these
purposes a descriptive case-control study was undertaken in which the indigenous
mothers were considered case subjects and the non-indigenous the control subjects.
The study took place in six municipalities of Ceará where there are health teams from
the National Health Foundation and took into account a sample of 165 indigenous
mothers and 148 non-indigenous. For the data collection, semi-structured interviews
were conducted, referring to the period January to October of 2009. Information was also
obtained through the information cards of the pregnant women as well as live-born and
infant death certificates. The data was analyzed with the Stata v.7 software. The
proportions were compared through the Chi-square test and Fisher's exact test. The
research showed that the socio-demographic profile of the indigenous mothers and nonindigenous was fairly similar. The average age of the indigenous mothers was 24 (± 6,5)
and of the non-indigenous 25 (± 6,5), with most of the indigenous women (73,6%) and
non-indigenous (70,1%) having a stable relationship (value-p 0,487) and with an average
of 7 (Indigenous) and 8 (non-indigenous) years of study, without a formal occupation.
The indigenous mothers (82,8%) had better access to running water than the nonindigenous (70,8%), value-p = 0,012. It was observed that the indigenous mothers
started the antenatal care later than the non-indigenous (52,5% versus 38,6%, value-p =
0,015). Level 1 of Coutinho classified the prenatal care of the indigenous women
(63,3%) and non-indigenous (50,3%) as intermediate, however with 2,5% of the
indigenous women and 1,5% of the non-indigenous with inadequate prenatal care
(value-p = 0,041). According to the results, it was concluded that the socio-demographic
characteristics and the quality of prenatal care given are very similar for both groups, the
only difference being that the start date of prenatal care and the number of appointments
had worse results among the indigenous mothers. No differences of statistical
significance were observed in the maternal and perinatal final outcome. In conclusion,
we can say that the quality of prenatal care and the obstetric and perinatal outomes are
very similar between the indigenous population and the non-indigenous patients of the
SUS in the State of Ceará.
