Cuidado clínico de enfermagem em saúde mental: contribuições da psicanálise para uma clínica do sujeito

Resumo:
A partir de observações acerca do cuidado de enfermagem às pessoas em
sofrimento psíquico nos serviços de saúde, objetivamos analisar as possibilidades
de um cuidado clínico de enfermagem pautado pelo referencial teórico da
psicanálise. Apostamos no fato de que, através da consideração da dimensão ética
do sujeito articulado ao seu desejo, podemos reinventar novos espaços e
ferramentas de atuação da enfermagem psiquiátrica. A psicanálise é o principal
referencial teórico dessa pesquisa, articulando a teoria à experiência clínica. Dessa
forma, debruça-se nas discussões da clínica psicanalítica para Sigmund Freud e na
releitura dessa clínica por Jacques Lacan. O percurso metodológico se deu através
da concepção de construção de caso clínico. Esse método trata de situar o sujeito
como verdadeiro operador, sendo que ao profissional/pesquisador cabe a função de
colocá-lo em condições de sê-lo. Assim, a construção do caso clínico implica em
fazer falar, trazendo para a cena discursiva o sujeito que sofre, reconhecendo que
seu sofrimento se articula ao modo que sua estrutura psíquica se organizou. Desta
forma, a construção do caso clínico inicia-se no momento em que o profissional se
prepara para escutar as demandas do sujeito e assim começar a delimitar, junto a
ele, o seu desejo inconsciente, a partir da demanda que este faz. O primeiro passo
foi o atendimento clínico dos sujeitos com queixas psíquicas no Centro de Atenção
Psicossocial da Regional IV de Fortaleza-CE, num período de seis meses, no
primeiro semestre de 2009. Só depois se passou ao estágio de construção do relato
do caso. As etapas dessa construção do caso consistem em: escrita do “pathos-doença” e a descrição da história da doença - escrever a clínica analítica do
“patho-doença”, através da mera descrição da história da doença; escrita do
“pathos-transferência” do paciente no tratamento analítico - nesse momento o
destaque recai sobre a mera descrição da história da “paixão-transferência” do
paciente em relação ao analista ao longo do tratamento analítico; escrita da
construção teórica - trata-se de escrever a análise e a interpretação das histórias
da doença e da transferência para se realizar a construção teórica em psicanálise.
Entendemos que são muitas as vias que se abrem nesse encontro da enfermagem
com a psicanálise. Na maioria delas, não se trata de fazer do enfermeiro um
analista. Isso nem mesmo seria possível dessa forma. Mas mesmo assim, há aí uma
contribuição, no momento em que se começa a entrar na ética do “um a um”, na
consideração do “caso a caso”. Dessa forma, exerce-se uma verdadeira “clínica do
sujeito”, onde o que importa é exatamente aquilo que do coletivo escapa, ou seja, o
cuidado com a singularidade. Finalmente, salientamos a importância dessa clínica
do sujeito na construção de novos espaços que façam emergir a fala do sujeito.
Almejamos, portanto, que o cuidado clínico de enfermagem em saúde mental tenha
como objeto de sua prática clínica o sujeito inconsciente e suas relações com a
emergência da fala.

Abstract:
From the observations on nursing care to people in psychic suffering in health
services, we aimed to analyze the possibilities of a nursing clinical care based on the
psychoanalysis theoretical reference. We believe that through the consideration of
the ethical dimension of the subject articulate to his desire we can reinvent new
spaces and performance tools of psychiatric nursing. Psychoanalysis is the main
theoretical reference of this research, articulating theory to clinical experience. It
enters in discussions of the psychoanalytical clinic for Sigmund Freud and in the new
reading of this clinic by Jacques Lacan. The methodological course happened
through the conception of the clinical case construction. This method puts the subject
as true operator, and the professional/researcher has the function of putting him in
conditions of being himself. The clinical case construction implies in speaking,
bringing into discursive scene the subject that suffers, recognizing that his suffering is
related to the way his psychic structure was organized. The clinical case construction
begins when the professional prepares itself to listen to the subject's demand and
then begin to delimit with him his unconscious desire, based on the demand that he
has. The first step was the clinical assistance of the subject with psychic complaints
in the Psychosocial Care Center of the Administrative District IV of Fortaleza-CEBrazil, in a period of six months, in the first semester of 2009. Only later we moved to
the stage of case report construction. The stages of this case construction consist in:
writing of "pathos-disease" and description of disease history - to write the
analytical clinic of "patho-disease", through the simple description of the disease
history; writing of "pathos-transfer" of the patient in analytical treatment - at this
moment the attention is on the description of the history of the patient's "passiontransfer" regarding the analyst along the analytical treatment; writing of theoretical
construction - to write the analysis and interpretation of the disease and transfer
histories to perform the theoretical construction in psychoanalysis. We understand
that there are many possibilities that appear in this nursing- psychoanalysis
encounter. In most of them is not about making the nurse an analyst, this wouldn't
even be possible this way. But even so there is a contribution there, in the moment it
enters in the ethics of "one to one", in the consideration of "case to case". A true
"clinic of subject" is exercised, where what matters is exactly that that escapes from
the collective, in other words, the care with singularity. Finally, we pointed out the
importance of this clinic of subject in the construction of new spaces that make the
subject's speech emerge. We hope that the nursing clinical care in mental health has
as object of its clinical practice the unconscious subject and its connections with the
emergency of speech.
