Distribuição espacial da mortalidade infantil em Fortaleza-CE no triênio 2006-2008: análise com base nos sistemas de informações em saúde

Introdução: O Coeficiente de Mortalidade Infantil em Fortaleza, apesar do declínio,
tem distribuição desigual nos espaços que compõem as Secretarias Executivas
Regionais (SER) quanto aos seus componentes neonatal, pós-neonatal e causas, o
que apontam a necessidade de melhor compreensão desse acontecimento. O
presente estudo tem por objetivo analisar a distribuição espacial da mortalidade
infantil por Secretarias Executivas Regionais do Município de Fortaleza-CE, no
triênio 2006-2008. Metodologia: Estudo transversal de base populacional,
descritivo, baseado em dados coletados do SIM e SINASC da Secretaria Municipal
de Saúde de Fortaleza-CE. Foram registrados 116.128 nascidos vivos e 1.842 óbitos
infantis em Fortaleza-CE, no período 2006-2008. Foram calculados os coeficientes
de mortalidade infantil, neonatal, neonatal precoce, neonatal tardio, pós-neonatal e
por causas principais de acordo com SER de Fortaleza, para o período 2006-2008 e
para cada ano do período. Para estudo da distribuição espacial foram utilizados
mapas temáticos por SER, construídos através do software ArcView 3.3, em seguida
fez-se análise frequencial e média. Resultados: No triênio 2006-2008, o CMI em
Fortaleza acompanhou a tendência nacional de queda, com variação de -12,8% na
taxa, passando de 17,26 em 2006, para 15,05%0 NV em 2008. Os óbitos em
menores de um ano tiveram como principal contribuinte as mortes neonatais, as
quais além de responderem por um elevado percentual (67%) dos óbitos infantis
ainda reduzem lentamente, com variação de -9,8% no CMN, em comparação com
as mortes pós-neonatais, cuja variação foi de -18,5% no CMPN. Houve redução do
CMNP de 7,8% e do CMNT de 16,9%. A SER VI deteve as mais baixas taxas de
mortalidade infantil de Fortaleza com registro em 2006 de 13,72%0 NV e em 2008 de
12,42%0 NV. O componente neonatal da mortalidade infantil teve menor média de
registro na SER VI (6,64%0 NV) e maior nas SER I (10,79%0 NV) e SER IV (10,45%0
NV). A recordista das mortes neonatais precoces foi a SER II com um registro médio
no CMNP de 8,24%0 NV e aumento de 8,36%0 NV em 2006 para 9,04%0 NV em
2008. Na mortalidade pós-neonatal houve aumento na SER I e SER II e redução no
restante das regionais. A principal causa da mortalidade infantil em Fortaleza foi por
afecções perinatais (8,84%0 NV) sendo responsáveis por 55,8% dos óbitos em
menores de um ano, seguidas pelas más-formações congênitas (18,46%), as
doenças infecciosas e parasitárias (7,87%) e as doenças respiratórias (5,59%).
Conclusão: Os resultados permitiram identificar SER onde a mortalidade infantil,
seus componentes e causas são mais evidentes. Os registros encontrados atendem
à meta anual de redução de 5% da mortalidade infantil, estabelecida no Plano
Municipal de Saúde de Fortaleza 2006-2009, apenas no período (2006-2007). A
estratificação dos componentes da mortalidade infantil por SER revelou a
necessidade de intervenções mais eficazes em toda cadeia assistencial de vigilância
epidemiológica.

Abstract:
Introduction: Infant Mortality Rate in Fortaleza, despite the decline, is the unequal
distribution in the spaces that make up the Regional Executive Secretariats (SER)
and its components, neonatal, post neonatal and Causes, which pointed out the need
for better understanding of that event. This study aims to evaluate the spatial
distribution of infant mortality by the Executive Secretaries of the Regional
Municipality of Fortaleza, in the triennium 2006 to 2008. Methodology: A crosssectional, descriptive, based on data collected from the SIM and SINASC the
Municipal Health Department of Fortaleza. Enrolled 116.128 live births and infant
deaths 1.842 in Fortaleza, in the period 2006-2008. Were calculated for infant
mortality, early neonatal, neonatal, late neonatal, post-neonatal mortality and main
causes according Being of Fortaleza, for the period 2006-2008 and for each year of

16.9%. SER VI He paused as lower Infant Mortality Rates in Fortaleza in 2006 with
record of 13.72% at 0 and NV 2008 from 12.42% 0 NV. The neonatal component of
infant mortality had lower mean in SER VI (6.64% NV 0) and higher in SER I (10.79%
NV 0) and BE IV (10.45% 0 NV). The record of early neonatal deaths was the SER II,
with an average record of 8.24 in CMNP NV 0% increase and 8.36% 0 NV in 2006 to
9.04% 0 NV in 2008. In the post-neonatal mortality was increased in BE I and BE II
and reduction of regional not rest. The main cause of infant mortality in Fortaleza
were perinatal conditions (8.84% NV 0), accounting for 55.8% of deaths in children
under one year, followed by congenital malformations (18.46%), as Infectious and
Parasitic Diseases (7.87%) and respiratory diseases (5.59%). Conclusion: The
regional results have identified where an infant mortality, its components and causes
are more evident. Records found meet the annual target of 5% reduction in infant
mortality, without Established Municipal Health Plan 2006-2009 in Fortaleza, in the
first period (2006-2007). The stratification of the components of infant mortality to be
revealed a need for more effective interventions in the entire chain of care
Epidemiological Suerveillance.
